Program Coordinator
Coordinate individual plans, support DSP teams and keep quality high across a caseload of the people we support.
The Program Coordinator holds things together. You will oversee individual plans for a caseload of the people we support, make sure the support described on paper is the support actually happening, and be the person families and service coordinators can rely on to pick up the phone.
It is a role for someone organized who also genuinely likes people. You will spend as much time listening as you do writing, and the quality of your relationships with families and staff will matter more than the tidiness of your files.
What you will do
- Develop, review and maintain individual plans
- Coordinate with service coordinators, LIDDAs and health professionals
- Supervise and support DSP teams
- Monitor quality and follow up on concerns quickly
- Keep accurate records and meet program requirements
- Be a reliable point of contact for families
What we are looking for
- Bachelor’s degree in social work, psychology, human services or a related field, or equivalent experience
- Experience supporting people with intellectual or developmental disabilities
- Strong written and verbal communication
- Well organized, able to manage a caseload and meet deadlines
- Valid Texas driver’s license and a reliable vehicle
- Able to pass a background check
