This policy explains what information Serenity Homes Wellness collects through this website, why we collect it, who we share it with, and what choices you have. It applies to this website only. Information we handle as part of delivering services to a person is governed separately, under the rules of the HCS program and our agreements with the State of Texas.

We have written this in plain language on purpose. If anything here is unclear, please ask us and we will explain it.

Who we are

Serenity Homes Wellness provides Home and Community-based Services supports in Austin and Central Texas. Our office is at 315 Doodle Ln, Hutto, TX 78634. You can reach us at info@serenityhomeswellness.com or (512) 643-3050. We are the controller of the information described in this policy.

What we collect, and why

We collect only what we need in order to reply to you. There are three forms on this website, and each one asks for something different.

Contact form.

  • Your name, so we know who we are speaking to
  • Your email address and phone number, so we can reply the way you prefer
  • Your preferred contact method and the reason for your inquiry, so it reaches the right person
  • Your message
  • Your consent to be contacted about the inquiry

Referral form.

  • Your name, organization, role, email and phone, as the person making the referral
  • Your relationship to the person being referred
  • The person's first name and last initial only, plus their city or ZIP code
  • A general service interest, preferred contact method and best time to reach you
  • A non-diagnostic indication of current program status and how soon support is needed
  • Your confirmation that you have consent, or the legal authority, to share these details

Careers form.

  • Your name, email, phone and city
  • The role you are interested in and your availability
  • A summary of your relevant experience
  • Your resume, if you choose to upload one
  • Your confirmation that you are authorized to work in the United States
  • Your consent to be contacted about your application

We also collect basic technical information automatically when you visit, such as the pages you view, the approximate region you are in, the type of device and browser you use, and the site that referred you. This is used to understand how the website is performing.

What we deliberately do not collect

None of the public forms on this website ask for a diagnosis, medical records, a Medicaid number, a Social Security number, a date of birth, banking details or a driver license image. The fields simply do not exist, so this information cannot be submitted by accident.

Please do not send any of that to us through this website. If you need to share detailed information with us, we will arrange a secure way to do it.

How we use your information

  • To reply to your inquiry, process a referral, or consider a job application
  • To arrange and provide the services you have asked about
  • To keep records of inquiries and referrals as part of running our business
  • To understand how the website is used and improve it
  • To meet legal, regulatory and program obligations

We do not sell your information. We do not share it with anyone for their own marketing. We do not use it to make automated decisions about you.

Who we share it with

We share information only where it is necessary, and only with organizations that process it on our instructions:

  • Our website hosting provider, which runs the site
  • Our database provider, which stores form submissions
  • Our file storage provider, which holds uploaded resumes
  • Our email delivery provider, which sends confirmations and internal alerts

Each of these processes information for us and for no other purpose. We may also disclose information where the law requires it, where it is needed to establish or defend a legal claim, or where there is a genuine risk to someone’s safety. If our business is ever transferred to another organization, information may transfer with it, and we would tell you before that happened.

Analytics and cookies

We use website analytics to see which pages are visited, how people arrive at the site, and which links are used. This helps us make the website more useful.

Analytics is configured so that the contents of forms are never sent to it. Names, email addresses, phone numbers, messages and anything health-related stay out of analytics entirely.

This website does not use advertising cookies and does not track you across other websites. We use only the cookies needed to make the site work and to measure its performance. If that ever changes, we will update this policy and ask for your consent first where the law requires it.

Most browsers let you block or delete cookies in their settings. Blocking essential cookies may stop parts of the site working.

How long we keep information

We keep information for as long as we need it for the purpose it was given, and for as long as our record-keeping and program obligations require. In practice that means inquiries and referrals are kept while we are in contact with you and for a reasonable period afterwards, and job applications are kept while we consider you for the role and for any similar role that comes up shortly after.

When information is no longer needed, we delete it. You can ask us to delete yours sooner using the contact details below.

How we protect it

  • The whole website is served over an encrypted HTTPS connection
  • Information submitted through our forms is encrypted in transit
  • Access to form submissions is limited to authorized staff who need it
  • Staff have individual accounts, with permissions matched to their role
  • Uploaded resumes are stored privately and are not publicly reachable

No website can be guaranteed completely secure, which is why we ask you not to send sensitive personal or medical information through this website.

Your choices and your rights

You can ask us at any time to:

  • Tell you what information we hold about you
  • Give you a copy of it
  • Correct anything that is wrong or out of date
  • Delete information we no longer need to keep
  • Stop contacting you

Depending on where you live, you may have additional rights under state privacy law, including the right to appeal if we decline a request. If you are in Texas, you can also raise a concern with the Office of the Texas Attorney General.

A note about health information

This website is not a patient portal and is not covered by HIPAA. Nothing you send through these forms should be treated as a protected health record. If you become a client, information handled as part of your services is subject to separate protections, and we will explain those to you at that point.

Children

This website is intended for adults. We do not knowingly collect information directly from children through it. Where a referral concerns someone under 18, we ask that it is made by a parent, guardian or an authorized professional. If you believe a child has given us information directly, contact us and we will delete it.

Links to other websites

This website links to external resources, including official State of Texas pages. Those sites are not under our control and have their own privacy policies. We would encourage you to read them.

Changes to this policy

If we change this policy we will update the review date shown at the bottom of this page. Where a change is significant, we will make that clear rather than rely on you noticing the date.
