For service coordinators and LIDDAs

For referral partners

Information for service coordinators, LIDDAs and community professionals.

You need to know two things about a provider before you refer: whether they have capacity, and whether they will do what they said. We try to make both easy to establish.

Send us a referral and you will hear back within one business day. If we have capacity, we will tell you what we can offer and when. If we do not, we will tell you that just as quickly. An unanswered referral helps nobody, and a provider who says yes to everything is a provider you will be chasing later.

Once someone is with us, you will have a named point of contact who knows the person and answers their phone. We will keep you informed of anything significant rather than waiting for a review meeting.

What you can expect from us

  • A response within one business day

Every referral is acknowledged quickly, including the ones we have to decline.

  • Honest capacity information

We will tell you plainly what we can take on and when. We do not hold referrals open to look available.

  • A named point of contact

You will know exactly who is handling a referral and how to reach them directly.

  • Minimal information up front

The referral form asks only for what we need to make contact. Detailed information comes later, through the proper channels.

  • Straightforward communication

If something changes or goes wrong, you hear it from us early rather than discovering it at a review.
